Care members using a network of contracted providers. </p><p>Responsibilities associated with the Housing Initiatives team include analysis of housing data on enrollment, payment, or claims data to identify operational problems or issues. Make recommendations to resolve problems and/or improve operations and quality of the housing program. </p><p>Community Health Platform: The Community Health Platform team supports and develops the infrastructure upon which Community Health initiatives operate.
