We are a national leader in transitional care management and its impact on unnecessary hospital readmissions. Our department’s clinical volume includes over 100,000 annual visits and over 2,500 hospital discharges across our 5 current clinics and 2 hospital services.

These activities, plus departmental leadership in population medicine, practice-based research network, and academic programs afford opportunities for faculty to participate in a variety of innovative programs. We offer competitive compensation based on rank and experience and an excellent benefits package that includes health, dental and life insurance; disability and malpractice insurance; a generous retirement plan; continuing education; paid time off and more.
