As an PH Developmental Specialist within the Children's 1st Program.At District Two, Georgia Department of Public Healthyou will join a dedicated group of public health professionals that help support the overall mission and vision of protecting lives of Georgian residents in a variety of ways. District 2 Public Health serves 13 counties in northeast Georgia, including Banks, Dawson, Forsyth, Franklin, Habersham, Hall, Hart, Lumpkin, Rabun, Stephens, Towns, Union, and White counties. One of the fastest growing regions in the state, District 2 includes rural and urban communities with diverse public health challenges.

Job Responsibilities

Job Description:
Under general to limited supervision provides professional nursing or community health services for infants and children in a specified geographic area. Provides developmental screenings, patient education and counseling, advises caregivers, and makes necessary referrals for other social services.
Additional/specific duties:
1. Provide developmental screening to all children, birth to age five years who have not been referred with a previously completed developmental screening.
2. Provide home visits to include assessments, parent education, developmental monitoring and developmental screening according to Children 1st model of service delivery. 
3. Refer identified infants and children to public health and community programs as appropriate. 
4. Link all foster care and Child Abuse Prevention and Treatment Act (CAPTA) children to health check;
5. Ensure that all children receiving services are linked to a primary healthcare provider.
6. Assure that all identified families receive Children 1st Maternal Child health (MCH) Integrated assessments according to programmatic guidelines. 
7. Refer all children with confirmed hearing loss for a genetic evaluation. 
8. Complete Healthcare Insurance Claim Form (HICF) 1500 for eligible services provide to children enrolled in Medicaid according to DSPS guidelines. 
9. Complete all necessary documentation, both written and electronic, and reporting requirements by specified deadline or as determined by Children 1st District Coordinator. 
10. Enter follow up actions, assessment, etc. into SENDSS NB.
11. Check SENDSS and/or maintain a tickler system to monitor children assigned.

Minimum Qualifications

Minimum Experience & Qualifications:
Master's degree and eligibility to be licensed
Or
Bachelor's degree in a related field and two years of experience in social service delivery.
 
Preferred Qualifications:

  • Georgia license in Social Work (LMSW, LCSW), Professional Counseling (LPC), BSN Registered Professional Nurse (RN)
  • Three to five years of experience in the provision of direct maternal, child and infant services in health care setting; OR
  • Three to five years of experience in the provision of direct care in a Neonatal or Pediatric Intensive Care setting; OR
  • One to two years experience in providing comprehensive in home assessments for mothers, infants, and children, or work experience in social services delivery (case management, outreach, prevention/education, etc.); OR
  • Knowledge of or experience delivering services via care coordination and or home visiting.
  • Experience with pediatric population and developmental milestones required.
  • Must be willing to travel.
  • Speak, read and write Spanish
