Job Responsibilities

Under general supervision, supports the Diabetes Prevention and Management program and works collaboratively in an organized way to achieve reductions in the burden of chronic disease and diabetes in Georgia. Responsible for providing ongoing, technical, and training support to Public Health Districts and Public Health partners, specifically around diabetes control and prevention.  Works collaboratively with the Diabetes Program Manager to support the promotion of evidence-based management and prevention programming and best and promising practices. Ensures stakeholder engagement in carrying out program activities. Shares best practice research and results from existing programs with stakeholders and maintains stakeholder engagement. Reports to the Diabetes Prevention and Management Team Lead. 

Management

This position requires independent program management and the ability to make decisions to improve program and service efficiencies. 

Uses project management tools and techniques to manage projects and activities related to Office programs and projects.  

Provides day-to-day administrative oversight of diabetes activities with a focus on diabetes prevention- specific tasks including grant management of partnering organizations and contractors who will assist with the development and implementation of activities.

Collaborates effectively with staff above, below, and across the organizational structure and Department to ensure the successful execution of projects and ideas.

Collaborates with other programs and sections within the Department, as well as other relevant government agencies, to ensure coordination of joint efforts of diabetes-related programs. 

Seeks input and engages others to create a positive and team-based environment that contributes to overall chronic disease prevention efforts for the State of Georgia.     

Manages interactions with prevention and health promotion partners, as directed, to achieve outcomes and goals.

Leads and organizes meetings with diabetes and family healthy weight program partners pertaining to contract needs. 

Programmatic

Builds and maintains key external relationships with a wide range of diabetes prevention and family healthy weight program stakeholders, including managed care organizations, public health districts and health systems. 

Uses creative and strategic thinking skills to find new and innovative ways to engage with stakeholders while furthering diabetes prevention and family healthy weight program activities.  

Participates as a diabetes prevention content expert in interdisciplinary/interdepartmental meetings as needed. 

Oversees the planning, implementation and evaluation of Diabetes Prevention and Family Healthy Weight programs.  

Communicates about program development and implementation to a wide range of stakeholders. Tailors presentations/engagement activities for various audiences such as healthcare/public health organizations, business groups/employers, academic institutions, legislators, and major stakeholders.  

Supports the development of written assessments of partner needs for programs.

Applies public health knowledge to develop analyses of programmatic and intervention needs in communities related to a particular program area. Designs services that incorporates population needs to expand availability and access to Diabetes Prevention and Family Healthy Weight programs.

Creates proposals for new programs and approaches to program implementation.  Engages partners in the development of those proposals, and identification of new programmatic goals related to diabetes management education.

Performs other duties as assigned by Manager consistent with professional training, experience, business goals and functions.

Minimum Qualifications

Bachelor's degree in a related field from an accredited college or university and four (4) years of experience performing work related to the area of assignment
OR
one (1) year of experience at the lower level Healthcare Prgm Consultant 2 (HCP091) or position equivalent.
Note: Some positions may require licensure.

Preferred Qualifications

  • A minimum of 2 years’ experience in health education or clinical management related field. 
  • Mastery of basic computer skills.
  • Assists in assessing needs, design and implementation of new, innovative Diabetes Self-Management Education course offerings, especially online, app-based, and other technology-based course formats 
  • Masters of Public Health degree or other applicable field of study (MPH or M.ED Instructional Design)
  • Experience in diabetes prevention, diabetes management, obesity interventions, or community initiatives is highly desirable.
  • Trained National DPP Lifestyle Coach.

DPH accepts educational credential recognized by the Council for Higher Education Accreditation (CHEA) and/or the US Department of Education (DOE) will be considered. DPH will contact educational institutions to verify degree, diploma, licensure, etc.
