Role: The Clinical Liaison Nurse is responsible for facilitating referrals in facilities as the Organization’s representative.

Qualifications:

  • Current license as RN in the state the employee is providing care, current active single-state out-of-state nurse license, or current active multistate/compact nurse license
  • Minimum of one (1) year of acute healthcare experience; hospice experience preferred
  • Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system 
  • Marketing or sales experience preferred
  • Able to perform admissions in the acute care setting
  • Ability to effectively communicate in English (written and verbal)
  • Excellent customer service focus
  • Detail oriented
  • Ability to multitask and prioritize
  • Ability to present oneself in a professional manner
  • Ability to manage physical demands and emotional stress of workload
  • Strong goal/outcome orientation with exceptional time-management skills
  • Mobile Driver - Valid driver’s license and automobile insurance per Company policy
  • Reliable transportation to meet schedule

Job Responsibilities:

  • Acts as the Organization’srepresentative at assigned facilities while facilitating referrals to all service lines; works closely with referring hospitals, physicians, facilities, patients, families and the general public.
  • Presents services in an empathetic and compassionate manner and performs admission process.
  • Obtains consents for services and arranges safe discharge in an efficient manner for referred and eligible clientsmeeting productivity expectations.
  • Assertively handles any barriers to admissions of referred patients.
  • Obtains medical record information for the referrals.
  • Develops strong relationships with case managers, physicians, etc. at facilities.
  • Works with Professional Relations Representatives (PRRs) to provide education on all available services.
  • Provides care for any GIP patients in a hospital including daily visits, professional management oversight and case management of hospitalized A & B patients, performs documentation and coordinates efficient discharge.
  • Communicates frequently with Patient Access and Care Connect staff to ensure timely follow-up.
  • Performs other duties as assigned.
