Description

Concern Housing is seeking a Director of Community Residence Programs who will be responsible for all aspects of the Port Jefferson and Shoreham Community Residences, as well as the Apartment Treatment community residence program, overseeing operations, as well as ensuring compliance with state and federal regulations.


Responsibilities Include but are not limited to:


  • Provide supervision for Port Jefferson and Shoreham Program Supervisors, CTAP Case Managers, and CTAP Peer Specialist.
  • Conduct weekly individual and group supervisory meetings.
  • Perform periodic staff evaluations, and participate in hiring and dismissing staff.
  • Oversee intake procedures, including screening applicants, collecting necessary documentation, arranging trial visits as appropriate, etc.
  • Closely monitor vacancies to ensure high occupancy rates.
  • Ensure compliance with OMH and Medicaid regulations and documentation requirements, including:
  • the provision of 6 services monthly to each client
  • Service Plans and Reviews according to due date requirements
  • the obtainment of Physician Authorizations according to due date requirements
  • Monitor and manage program A/R reports to ensure maximum revenue.
  • Collaborate with other agency departments as needed to ensure optimal operations, including Property Management, Accounting, and Human Resources.
  • Provide 24/7 crisis intervention support.
  • Assist with coordination of site coverage as needed.
  • Assisting staff and clients with issues related to securing and maintaining entitlement income.
  • Remain current with all Agency required compliance documents and trainings.
  • Other tasks as assigned by the Director of Long Island Program Operations.

Salary Range: $75,000 - $90,000

 

Position Requirements

Masters Degree in Social Work, Rehabilitation Counseling or other related field.  Minimum of five (5) years experience working in a residential setting with similar populations. Minimum of three (3) years of supervisory experience or training.
