Under the general direction of Specialty Care Clinic management, the HOPWA Housing Case Manager coordinates with Advantage Behavioral Health to provide housing assistance and related supportive services for persons living with HIV/AIDS and their families.
As a HOPWA Housing Case Manager within the Northeast Health District at the Georgia Department of Public Health, you will join a dedicated group of public health professionals that help support the overall mission and vision of protecting lives of Georgian residents in a variety of ways.
Job Responsibilities
You will support the Department by utilizing your expertise to fulfill the following responsibilities:
- Determines client eligibility and ensures program compliance
- Manages case load that includes conducting intake, screening, and assessment
- Collects and processes client documentation and forms
- Reviews client paperwork for completeness
- Maintains data entry and keeps files and reports up to date
- Coordinates client appointments, transportation and support services
- Ensures client compliance with HIV medical care and treatment
- Implements strategies for clients to avoid long-term dependence on HOPWA assistance
- Conducts off-site and home visits as needed
- Educates clients on rights and responsibilities
- Attends professional meetings, educational conferences, and in-service trainings required by Advantage and/or DPH in order to maintain and improve professional competence
- Coordinates with DPH and Advantage to maintain an enrollment list of clients that may qualify for housing or other forms of assistance
- Ensures that documentation and records are maintained in accordance with federal, state, and local guidelines.
- Assists in identifying and coordinating resources within the covered area
- Completes other duties as assigned
Minimum Qualifications
Master’s degree in a related area from an accredited college or university AND eligibility to be licensed.
Annual salary $45,000. Plus, generous benefits include 13 paid holidays, annual and sick leave, health insurance, and retirement plans.
