Job Summary

This position is a hybrid role, with a significant portion of the work being conducted through home visits as part of an innovative initiative focused on advanced Transition Care Management.

How this role integrates into patient care is essential, as the LPN will be a key member of the will be a key member of multidisciplinary teams, working collaboratively to ensure comprehensive patient care, working collaboratively to ensure comprehensive patient care. As an LPN, you will be instrumental in supporting patients as they transition between healthcare settings, providing essential continuity of care through home visits and other touchpoints, which promotes patient satisfaction and optimal clinical outcomes. By connecting with patients directly in their homes, you will help to proactively manage their healthcare needs, improving their overall experience and health trajectory.



Skills / Knowledge / Abilities

Care coordination, excellent communication skills, health education teaching skills, and triage experience is a plus.
