Job Responsibilities
Under general supervision, provides a broad range of paraprofessional social services to patients/clients/consumers and their families. Conducts patient/client/consumer intakes, participates in treatment team planning, and performs crisis intervention. Assists in dealing with personal and social problems. May provide supportive counseling to consumers and families and/or serve as a liaison for social services. May perform case management duties.
Primary Duties & Responsibilities:
- Assists clients in locating and utilizing community resources including legal, medical, financial assistance, and other referral services.
- Completes patient/client/consumer intakes based on interviews with patients/clients/consumers, their families, significant others, and appropriate community agencies.
- Counsel clients living in group homes and halfway houses; assists in pre-release and release planning relating to clinic services.
- Implements and organizes the delivery of specific social services within the community.
- Maintains contact with other social service agencies and health care providers involved with clients to provide information and obtain feedback on client’s overall progress.
- Maintains program statistics for purposes of evaluation and research.
- Monitors patient’s/client’s/consumer’s progress towards treatments goals and performs follow up at regular intervals.
- Prepares intake reports and case history records.
- Provides short-term case management and referral services to clients with emergency situations.
Minimum Qualifications
High School diploma/GED and two (2) years of job- related experience in a social services-related position; or One (1) year of experience at the lower level Social Svcs Tech 1 (SST010) or equivalent position.
Peer Advocate should be members of the HIV/AIDS community (PLWHA - People living with HIV/AIDS)
Preferred Qualifications:
- Basic computer skills (Microsoft Word & Excel experience)
- Access to reliable transportation
- Have appropriate skills, relevant experience, cultural and linguistic competency, knowledge about HIV/AIDS & client confidentiality and knowledge of available health and social services related resources
- Prior participation in consumer leadership training

All approved teleworking employees must reside within the state of Georgia and depending on the responsibilities of the position, must live within reasonable driving distance of the agency’s primary worksite and be able to report to the office when required.
Current State employees are subject to State Personnel Board rules regarding salary.
