The Southeast Health District Wellness Clinics are seeking two highly qualified candidate to fill the full-time, salaried positions of Social Service Specialist. One position will be located in Douglas or Waycross, GA and the second position located in Statesboro, GA

As a Social Service Specialist with the SEHD, you will join a dedicated group of public health professionals that help support the overall mission and vision of protecting lives of Georgian residents in a variety of ways.

Job Responsibilities

Under general supervision, performs a variety of case management services for the housing program (HOPWA).  Duties to include but not limited to completing housing assessments, assisting clients with developing and implementing permanent housing plans, maintaining files and EHR, providing case conferencing and referrals, assisting in developing community partnerships and resources, and completing reports on housing outcomes.

Minimum Qualifications

Master's degree in a related field from an accredited college or university AND eligibility to be licensed OR Bachelor's degree in a related field from an accredited college or university AND Two years of experience in social service delivery.

Preferred Qualifications:

Master's degree in a related field from an accredited college or university AND Two years of experience in social service delivery.  Experience working with HIV/AIDS patients.
