Job Responsibilities
- Provides outstanding customer service
- Maintains HIPAA standards related to Privacy and Security
- Provides a broad range of social services to individuals with HIV/AIDS and their families.
- Assesses to determine eligibility for Ryan White Program and enrolls and recertifies as appropriate. Develops plans and assesses effectiveness.
- Links to community resources for assistance. Participates in community consortium meetings.
- Collects and maintains data and completes and submits required reporting
- May work nights and weekends, participate in health fairs, serve on committees, and make presentations, or respond to disasters.
- Responds to Public Health Emergencies
Minimum Qualifications
Master's degree in a related field from an accredited college or university AND eligibility to be licensed OR Bachelor's degree in a related field from an accredited college or university AND Two years of experience in social service delivery. (Please mail or bring a copy of College Transcript or Diploma to Interview—must have official transcript prior to start date). Must be able to lift up to 45 lbs, stoop and bend.)

- Public Health experience
- Bachelor or Master degree in Social Work
- Experience working with individuals with HIV/AIDS
- ADAP experience
- Case management experience
- More than two years experience in social service delivery
- Experience using CareWare
