Job Summary
The UK HealthCare Population Health Team seeks a Social Worker Senior to work as part of a multidisciplinary primary care team spanning inpatient and outpatient to help coordinate services for patients transitioning out of the hospital setting. The primary location for this position will be in the inpatient setting and will work collaboratively with the inpatient case management team. The Social Worker Senior will know about environmental and/or psychosocial aspects that influence patient outcomes. This individual will assess the need for further support related to social drivers of health and coordinate with UKHC programs (inpatient and outpatient) and community-based organizations for positive outcomes. This role will involve building relationships with patients through trust, motivational interviewing and empowering patients to take an active part in their health.
Skills / Knowledge / Abilities
Good communication, organizational, and problem solving skills; Experience with Windows PC, Microsoft Office (Word, Excel, Databases); Understanding of social drivers of health and their impact on patients.
