Job Summary

The Bluegrass Care Clinic is seeking a dedicated Social Worker to serve as the Basic Needs Programs Lead overseeing emergency rent and utility assistance, long-term housing programming, and food bank and home-delivered meal services, specifically designed to support individuals living with HIV. These programs are funded by the Ryan White grant, which is essential in promoting positive health outcomes and enhancing the quality of life for community members through access to critical resources.

On a day-to-day basis, this position will supervise and mentor staff responsible for delivering these services, ensuring efficient service delivery and adherence to best practices. They will also support staff by engaging directly with clients to assess their needs and connect them with appropriate services. The supervisor will develop and implement processes to ensure seamless service delivery in collaboration with the multidisciplinary care team. They will conduct regular program evaluations to ensure that all programs are aligned with health-focused goals and are meeting grant requirements.
