Position Specific Duties/Responsibilities: <ul><li>Provided individual, group and family therapy / clinic services, at a school based satellite location</li><li>Assess each client’s mental health strengths and needs in accordance with the most current edition of the DSM / ICD 9 codes and in accordance with pertinent regulatory and agency standards</li><li>Ability to detect clients at risk, with the aim of preventing further psychological or physical deterioration</li><li>Effectively collaborate with the client’s continuum of care providers to include psychiatric care provider, primary health provider and other involved health care providers</li><li>Ensure full engagement of each client in the therapeutic process as evidenced by consistent attendance at scheduled sessions and contributions to treatment planning and progress</li><li>Complete and maintain all intake assessments, treatment plans, progress notes and all required documentation within the regulatory time frames and in accordance with professional standards, agency policy, Office of Mental Health guidelines and Federal Medicaid and HMO guidelines</li><li>Assure that each assigned client’s record is maintained with current to date information in accordance with pertinent regulatory and agency standards</li><li>Assist in 24/7 coverage of the clinic</li><li>Other duties as assigned</li></ul> Knowledge, Skills, and Abilities <ul><li>Excellent documentation skills</li><li>Ability to work independently and also be an enthusiastic team player</li><li>Ability to work evening hours to assist in 24/7 coverage of the clinic and/or meet with clients around their availability.</li><li>Ability to work nights. Assist the Medical Director in maintaining 24 hour, 7 day per week medical coverage for PACE patients in Hope Hospice facilities and services (if applicable). Provide on-call coverage for Hope Palliative Care patients/families on a rotation basis (if applicable). Participate in Hope PACE medical staff meetings and complete any mandatory continuing medical education activities as directed by the administration, in addition to CME needed to maintain certification. Participate in any Quality Assurance activities approved by the administration as appropriate to your job setting.
